Prescription Assistance

PRESCRIPTION ASSISTANCE HELPS ELIGIBLE VERMONTERS PAY FOR PRESCRIPTION MEDICINES.

What help is available?

There are currently two programs:

  1. Healthy Vermonters: provides a discount on prescription medicines with no monthly premiums.
     
  2. VPharm: helps pay for prescription medicines with affordable monthly premiums.

Who is eligible?

  1. Healthy Vermonters: individuals who meet the income guidelines AND have no prescription coverage or have reached their maximum benefit amount on their current prescription coverage.
  2. VPharm: Individuals who meet the income guidelines AND are enrolled in Medicare Part D coverage.

How do I apply?

  1. To apply for prescription assistance only:

    1. Complete the 201P Pharmacy Programs Application:
      Click here to print a copy or call 1-800-250-8427 and ask that one be mailed to you.

    2. Mail your completed application, along with copies of required supporting documents to:

      Application and Document Processing Center
      280 State Drive
      Waterbury, VT 05671-1500

  2. To apply for prescription assistance AND Medicaid for the Aged, Blind, or Disabled (MABD):

    1. Complete the 205ALLMED non-LTC by hand or as a fillable form:

      1. Complete it by hand:
        Click here to print a copy of the form or call 1-800-250-8427 and ask that a 205ALLMED non-LTC be mailed to you. Fill it out completely. Be sure to sign and date it.

    2. Mail your completed and signed application, along with copies of required supporting documents to:

      Waterbury, VT 05671-1500


How do I renew?

1. Complete by hand the 201P Review renewal form sent to you in the mail. Use the envelope with the green stripe included in the renewal packet. OR

2. Fill out the 201P Review health benefits renewal form on your computer. Print AND sign your completed renewal form. You can either:

  • MAIL your completed AND signed renewal form, along with copies of required documents to:

3. Call us at 1-855-899-9600. Monday-Friday 8am-4:30pm.

4. Contact an in-person Assister to help with the renewal form.

If we don't get the renewal form by the deadline, your health benefits will end.

How do I apply for other health care programs?

  2. Medicaid for the Aged, Blind and Disabled (MABD Medicaid).
