Notice of Decision - RE005EP
This notice is sent to customers whose Medicaid or Dr. Dynasuar has been automatically renewed. It means we have enough information about your household to complete your renewal. You do not need to take any further action.

Medicaid Renewal - 205IFAR
This notice is sent to customers enrolled in Medicaid for Children and Adults, including Dr. Dynasaur. You will get this notice if we need to verify financial or non-financial information. It means you are required to take action and respond. The information we have on file is already on the form. You need to change any wrong information AND enter any missing information. You also need to send copies of documents we need from you. We need the renewal form by the "Respond by" date circled in red. If we do not hear from you, your coverage may be re-determined, denied, or terminated.

Medicaid Closure - EE307
This notice is sent to customers enrolled in Medicaid for Children and Adults, including Dr. Dynasuar. You will get this notice as a reminder that you did not respond to the renewal notice. It is a reminder that you still need to take action to keep your health coverage. If you do not respond by the due date, you will lose your health coverage.

- Renew your coverage before it ends.
- Re-apply at any time for coverage. If you are not eligible for Medicaid, you could enroll in a Qualified Health Plan.

Medicaid Redetermination - 202MED Review
This notice is sent to customers enrolled in Medicaid for the Aged, Blind, and Disabled or a Medicare Savings Program. You will get this notice to confirm your eligibility to receives either of these benefits. It is a blank form. You will need to full out financial and non-financial information. If we do not hear from you, your coverage may be redetermined, denied or terminated.

Health Benefits Renewal (VPharm and Healthy Vermonters) - 201P Review
This notice is sent to customers enrolled in VPharm, a Medicare Savings Programs and/or Healthy Vermonters. You will get this notice to confirm your eligibility in these programs of other health benefits. It is a blank form. You will need to fill out financial and non-financial information. If we don't hear from you, your coverage will end.
