LTC Medicaid Overview
LTC Medicaid provides long-term care services and supports, for individuals residing in a nursing facility or community setting. Vermont’s LTC Medicaid Program includes Choices for Care (CFC), Developmental Disabilities Home and Community Based Services (DD HCBS), Brain Injury Program (BIP), and Intensive Home and Community Based Treatment (IHCBS).
To be eligible for LTC Medicaid, you must:
- Be a Vermont resident.
- Be at least 65 years of age or 18 years of age or older with a physical disability or eligible for Medicaid under Medicaid for Children and Adults (MCA) rules.
- Meet citizenship and identity requirements.
- Meet the Department of Disabilities, Aging and Independent Living (DAIL) clinical criteria including LTC provider and start date.
- Meet financial and non-financial criteria for LTC Medicaid.
