APPEAL

GRIEVANCE


If you disagree with a decision, then you can file an internal Appeal:

  • You may ask the Department who made the decision to review that decision.
  • It must be within 60 calendars days from the decision.
  • If you have been told your benefits or services are going to end or be reduced, you must ask that they continue during your appeal.
  • If you get benefits or services during your appeal and the appeal is not decided in your favor, you may have to pay for them.

To file a Fair Hearing:

  • You can mail a letter to:

If you want to file a Grievance:

  • You may file a grievance if you have a complaint about something other than an action. 
  • If you can't work out your differences with a provider, you may file a grievance.
  • The Department will send you a letter within 90 days about how they can address the issue.
