You may use the Document Uploader to submit supporting documents related to you application or renewal for any of the Vermont Medicaid programs including:
- Medicaid for Children and Adults (MCA) including Dr. Dynasaur for people under 65 years of age, children under 19 years of age, and pregnant persons.
- Medicaid for the Aged, Blind and Disabled (MABD) for people who are 65 years of age or older, and/or those who are blind or disabled.
- Long-Term Care Medicaid (LTC Medicaid)
- VPharm (Pharmacy Program)
- a Medicaid Savings Program (MSP)
- Disabled Children's Home Care (DCHC)
- the Healthy Vermonters Programs (HVP)
