We would like to invite you to participate in an online survey to better understand Vermont health and dental providers’ opinions and habits surrounding tobacco cessation counseling.*
The goal of the survey is to better understand critical issues and key factors impacting providers in the delivery of effective tobacco cessation support to patients. This research is being conducted by HMC Advertising, a marketing partner to the Vermont Department of Health. Please note, all survey participants remain anonymous and your contact information will in no way be linked to your survey answers.

* Note: tobacco cessation counseling is defined by the American Academy of Pediatrics as screening patients for tobacco use and secondhand smoke exposure, encouraging patients and families to stop smoking, and providing cessation strategies and resources, including pharmacotherapy.
