Health Benefits Exchange Planning and Implementation, Req # 03410-103-12 (10/27/2011)

The Department of Vermont Health Access is seeking to establish service agreements with one or more companies that can provide assistance in Vermont's second year of planning for design and implementation of federally mandated American Health Benefits Exchange.  Award of this bid is expressly conditioned on receipt of grant funding. Even if grant funding is awarded, the State expressly reserves the right to award no contracts pursuant to this RFP, in its entirety or in sections.  Contracts awarded pursuant to this RFP will not include minimum guarantees of funding and will include payments based on actual work performed as requested.

The states of Iowa, Maine, Oregon, Utah, Vermont, West Virginia, and Wyoming have formed a multi-state administered Medicaid drug rebate pooling arrangement, Sovereign States Drug Consortium (SSDC).  This arrangement, formed in 2005 with three founding member states has successfully grown to the current seven member states offering significant drug savings to all Medicaid beneficiaries.  The SSDC is seeking to establish a service agreement with one company/individual that can provide State Pharmacy Programs Oversight/Operations.  The successful bidder will be expected to provide support for the development, review, and execution of the Sovereign States Drug Consortium Memorandum of Understanding (MOU) including annual renewals along with oversight of the rebate procurement contract. The State of Vermont will act as the lead contracting agency, however all member states share in fiduciary and program responsibility.

Health-Care Related Tax Study, Req #03410-101-12 (9/9/2011)

 

The Department of Vermont Health Access, (DVHA) is seeking to establish service agreements with one or more companies that can assist in the development of systems and collection of data necessary to administer health-care-related tax. Work will include documentation of all steps involved to conduct the required work, coupled with a list of required data and information resources, and to submit a final written health-care-related tax work process and practice document to the State. The 2011 legislative session granted authority to the Secretary of Administration to explore the functionality and practicality of establishing any health-care-related tax under 42 C.F.R. part 433.50 et seq. which Vermont does not currently levy, including an analysis of the base to which such a tax would apply and mechanisms for collection. Under the Secretary’s authority administration of this project has been delegated to the DVHA. Any resulting contract(s) shall be written as a performance based contract and as such all submitted proposals shall be written with that structure.

 Full-Time Quality Improvement Practice Facilitators (08/11/2011)

The Expansion and Quality Improvement Program Facilitators work with multi-disciplinary teams in   practices to successful in implementing and managing quality improvement including National Committee on Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) recognition; effective use of information technology systems such as registries (Covisint DocSite) and portals to improve patient care; integration of self-management support, shared decision making, and planned care visits; redefining roles and establishing team-based care; and seamlessly connecting with community resources and specialty referrals (for example with the Community Health Team).

The Department of Vermont Health Access (DVHA), part of the Agency of Human Services (AHS), is responsible for the administration of health care services for those eligible for Medicaid, Dr. Dynasaur, State Children's Health Insurance Program (SCHIP), Vermont Health Access Plan (VHAP), Catamount Health and other publicly funded health insurance programs in Vermont. Green Mountain Care is the umbrella name for the state-sponsored family of low-cost and free health coverage programs for uninsured Vermonters (http://www.greenmountaincare.org/). DVHA provides medical assistance to nearly 26% of Vermonters. DVHA health care services are provided through managed care and fee-for-service delivery systems. The health care services system is a collaborative process that includes representatives from the provider, beneficiary, multiple State government agencies and the Centers for Medicare and Medicaid Services (CMS).DVHA began the enrollment of individuals into managed health care in October 1996. From the beginning, DVHA has contracted for the provision of independent unbiased benefits and enrollment counseling to those being enrolled into managed health care. In addition, since April 1998, DVHA has contracted for the provision of general member services for enrollees and others eligible for medical assistance. The current contract was effective starting July 1, 2005 and the last extension is scheduled to end October 31, 2011. DVHA is requesting proposals from qualified organizations to provide benefits counseling, enrollment services, outreach, and member services for health care beneficiaries.

The State of Vermont, Department of Vermont Health Access is issuing a Request for Proposal (RFP) on behalf of the SSDC.  The SSDC is a partnership of the states of Iowa, Maine, Oregon, Utah, Vermont, West Virginia, and Wyoming formed to secure rebates for their Medicaid pharmacy benefit programs.  The RFP is for services to solicit, negotiate, and procure Medicaid supplemental drug rebate bids and other Medicaid pharmacy benefit rebates for the SSDC member states.

Quality Improvement Practice Facilitators (12/22/2010)

The State of Vermont, Agency of Human Services (AHS), Department of Vermont Health Access (DHVA) is responsible for the Blueprint for Health, and has issued an RFP to contract for personal services for up to two full time practice facilitators.

The Blueprint for Health is building a supportive infrastructure of practice facilitators to develop the capacity and expertise of primary care medical practices to achieve systems transformation. 
Shifting the focus of primary care from episodic or acute care to prevention and the coordination of chronic disease care requires profound delivery system improvement and transformation leading to sustainable changes in clinical practice. Clinical practice improvement and transformation is complex and multifaceted enhancing clinical skills, shifting roles and responsibilities of staff, providing tools for clinical teams, patients and families, creating seamless connections between health care and community resources, and integrating Community Health Teams into primary care. The interaction of a patient with their medical home, before, during and after clinical visits will be affected.
